EACH APPLICANT MUST APPEAR IN PERSON with items 1-6 as listed below.
CHILDREN AGE 15 or younger MUST APPEAR IN PERSON WITH BOTH PARENT(S)
Parents of minor children must prevent their valid photo I.D. (original AND photocopy - front & back)
If only one parent appears you must also present evidence of:
- Sole legal custody of the child, or
- Second parent's notarized written statement, with copy of photo I.D. - (front and back) consenting to passport issuance for the child. (Form DS-3053), or
- Statement of Exigent/Special Family Circumstances (Form DS-5525) made under penalty of perjury explaining the second parent's unavailability.
Applications accepted 8:00 a.m. to 3:30 p.m. Monday through Friday (excluding State holidays)
Please obtain items 1-6 below.
- Application (form DS-11), completed in BLACK INK. Application forms are available online at www.travel.state.gov and in the Circuit Clerk's office.
- IDENTIFICATION: A government-issued photo I.D. such as driver's license or military I.D. for applicants age 16 or older. Please bring the original AND a photocopy (front & back side).
- PROOF OF CITIZENSHIP: (Listed below are the most common)
- CERTIFIED COPY of your Birth Certificate. (A certified copy will have a raised certification seal or colored ink certification seal by the State in which you were born) -OR-
- ORIGINAL Naturalization Certificate AND a photocopy of the naturalization certificate

- PHOTOGRAPH: One 2" x 2" passport photo, with plain light-colored background.
NOTE: Eyeglasses and caps/hats MUST be removed prior to taking photo. Also, eyes must be visible and face must not be obscured.
Due to the specific requirements for pose, attire, etc., we recommend that you obtain your passport photo from a professional photographer or a retailer who offers passport photo services.
- PASSPORT APPLICATION FEE: (Personal check or money order payable to U.S. Department of State) (no cash, credit cards or debit cards, and no checks drawn on a business)

- ACCEPTANCE AGENT FEE: Separate check or money order payable to Circuit Clerk. (We also accept cash for this fee only) (see www.travel.state.gov for current fees)
NOTE: The clerk will gladly make the required photocopies of your citizenship documents and drivers' license, if requested. There is an additional charge of 50 cents per page for any photocopies made by the clerk.
